Sound distortion on Macbook Pro mid-2009

Can anyone suggest why I suddenly got distortion on both speakers at the same time. It seems worse on lower frequency sounds/voices.
As it's not there with headphones or external speakers, it seems strange that it happened to both speakers at the exact same time.
Could it be a software problem with the sound card etc?

-Which version of OS X/macOS?
-Does the distortion happen in all applications, or just some?

If it doesn't happen with headphones/external speakers, the audio hardware is fine. It sounds like the speakers themselves may have been overdriven at some point. Or, there could be foreign matter/liquid damage to the internal speakers.
