Junk Mail

I am using Mac Mail and my junk mail has always gone to my Junk Folder. I recently lost the Mail Settings and had to reset them. Since doing so, my junk mail is not filtered and shows up in my Inbox. I have gone to Mail Preferences and clicked on all that should make this happen, but nothing is working. It is very annoying. Don't know what else to try.

-What version of OS X?
-What version of Apple Mail?
-What caused you to lose the previous settings?
-How may email addresses are setup?

- OS X 10.10.5
- Mail 8.2
- I could receive emails, but couldn't send them, so I checked the Mail Settings. Don't know how these suddenly were incorrect, but had to reset to the correct ones. I also updated OS X recently, so don't know if either of these caused the problem.
- Not sure about your last question. Do you mean how many email addresses are set up in my address book?

After resetting Junk Mail, I still continue to get 90% junk mail in my inbox. Don't know what else to try. Any suggestions?
