Updates don't work

I am still operating on MacOS Sierra 10.12.6 because new updates don't work for me. iTunes updates don't work automatically either. Every iTunes update has to be done manually. I've spent hours on the phone trying to sort this, but nobody at Apple has been able to solve this update problem.

Can anybody suggest a next step to finding a solution please?

-Which exact model Mac?
-Do you have a current Time Machine or other backup of your data?
-How much free space is available on the drive?
-Have you tried downloading and installing the macOS Sierra 10.12.6 Combo Update
