Safari making up and storing new, wrong passwords?

On my Macbook with Ventura 13.5.2 I noticed Safari is making up additional incorrect passwords for sites I visit. So if I have a password for somesite.com with the username John Smith it will add additional entries with wrong passwords. When I go to sign it, it shows lots of login options.
I have cut off autofill, but it's still happening and it is syncing to my iPhone as well. Ran a virus / malware scan on my computer. Nothing shows up. It's almost like it is making up strong passwords for every site and not stopping. I can erase them, but the process starts again.
  1. Any idea what is happening and why?
  2. Is this happening through iCloud or something to sync it to other devices?
  3. How do I stop this?
