Old podcast episodes show up in "Listen Now"

Sometimes I get a podcast episode showing up on my iMac, my iPhone, & my Apple Watch that I listened to months ago. If I right click on it on my Mac, one of the options is "Mark as unplayed", so it knows it's already been played. If I fast-forward through that episode, an even older episode shows up.

I removing the download. Finally, I just sit down on my iPhone, and fast-forward through a half-dozen podcast episodes until it stops, and then hope my devices sync. Right now my iPad shows dozens of podcasts that I have played with my Apple Watch.
