Rename finder item based on folder it came from

I have been searching everywhere but nothing matches exactly and most posts are old and I believe I am missing something with the variables portion of automator. I am looking to rename a pdf inside a folder called "output" that is inside a folder that is named job # and then client name. I get some of this to work by using path variable but I dont need the complete path.
Example:
65974 Client Name inside that folder are other supporting folder but I need the PDF that is inside the "output folder" that would be renamed form "65974 Part 1.pdf " to be "65974 Client Name_65974 Part 1.pdf" This file would then be copied to another hotfolder for our other systems. Any help would be greatly appreciated. Below is an actual file setup

No Automator can't do this, you'll need to look at AppleScript.
