iMac won't boot

Late 2009 iMac running High Sierra 10.13.6

I can't seem to get my iMac to boot. It hangs at the loading bar every time I boot. I can boot into recovery, safe mode, and single user mode. I cannot boot into Apple Diagnostic mode.

What I've tried:
- Reinstall from recovery
- Reset PRAM
- Make bootable usb using Transmac
- Make bootable usb using createinstallmedia method in safe mode
- Reinstall from recovery booted from usb
- Full wipe and repartition of main drive then reinstall from usb recovery

Installation seems to work alright but it still hangs at the loading bar. If I boot into safe mode after the installation it seems to finish the setup but still won't boot normally. Are there any other solutions I could try?

Sorry to hear about your issue. When you Erased and formatted the internal drive, were there any warning/error messages from Disk Utility?

No warnings/errors were displayed. I deleted the partition, reformatted to make sure it was clean, then reinstalled the OS on it.
