Need help dual booting Ubuntu/ Mac OS X

Hello everyone, I recently created a bootable Ubuntu USB drive which I used to install Ubuntu onto a separate partition of my internal disk. I used disk utility to create the partition and I had no problems installing Ubuntu until it came to installing a boot loader that was able to recognize my Ubuntu partition. The problem is, when I tried to install reFind last time I ended up somehow completely erasing my Mac HD which I just recently recovered. I still have the partition that I made before I deleted all of my data, but I'm just not sure what to do to be able to boot into my new partition. I have included a picture that may help.

Hello and welcome.

I don't have any experience with a dual boot OS X/Ubuntu Mac setup. I have seen issues with rEFInd before, so it is always recommended to have a current Time Machine or other backup of your data before attempting an install/setup of this kind.
