command+tab suddenly stopped working!

Its so annoying, i can't figure out what I did. Iv'e set the keyboard back to default in system prefs and also changed keyboad to check that wasn't the issue.

Any of you guys come across this before? I must use the shortcut about 100 times a day and its driving me crazy!
