file names appear on top margin of hard drives--how to delete them?

This is a weird question, and I can't find the answer online because I probably am using the wrong terms to search.

But on several of my hard drives I have a file icon in the top grey border area where you have "view" "arrange" "action" etc. The file is "live" in the sense that if you click it, it opens. But I can't get rid of it (them, really since this has happened on several drives) by dragging. It won't drag. And of course I have no idea how the files got in that border area originally. Any ideas?

It looks like this--

where the "?" have somehow appeared and a MPEG file appears at the far right. This is on multiple external hard drives.

Hello,

-What version of OS X?
-Have you tried: Finder > View > Customize Toolbar... > ...or drag the default tool set into the toolbar.?
