Export Table of Contents to PDF using Pages

I've been following the instructions here to add a table of contents to my document.
However, when I export it into a .pdf file, I can't see the sections generated when opening the TOC sidebar either in Preview or Firefox web browser.
Anything I am missing you can help me track down?
Another question:
How can I generate a TOC in Pages, but not see it at the first page of the document? I don't want the list to appear, I just want it to be in the "meta" information of the file.

-Which version of Pages?
-You are using Paragraph Styles, correct?

Yes, I use the paragraph styling to generate my TOC (I guess).
