ok looks like it's a pdf.

Finder > File > Get Info on the file, change its extension to pdf and see what preview has to say when you open it.

If it's a Form you will need Adobe PDF Reader, but when you install that make sure you don't let it become the default reader, Preview is heaps faster and better!
