Powermac G5 keeps reseting itself back to default..

I have a powermac G5. The problem that I am having is the computer keeps reseting itself back to default. And I lose everything. It happens when the computer freezes for a moment then it asks about the keychain default. I always select cancel. But as soon as i reboot my computer it resets. I lose all of my songs out of itunes, all of my settings return to default. It keeps happening. If anyone can help I would greatly appreciate it.
