cd won't mount or eject

I have a MacBook Pro laptop, and now there's a cd-rw (blank, standard size cd) stuck in the drive.

I tried eject using itune command, eject using eject button, tilting and pressing the eject button, terminal window with drutil tray eject command, and most of RIC's suggestions that i can find i've tried already but disk is still stuck.

I tried restart why holding down pad sensor, but it just looped and won't restart, so i had to press the power button and hold until it power down completely.. but still no luck...

I tried this one again..
open disk utility, then select both HL-DT-ST DVDRW GS21N and then press the Eject from the top bar

then i got this message
Eject failed
The disk "HL-DT-ST .....GS21N" could not be unmounted.
make sure that all application and files are close on this disk.

And yes all application are closed, i made sure by using force quit and quitting everything...

Please please please can someone help me..

i'm so frustrated and will probably smash this cd to pieces when I get my hand on it again! grrrr
